Cancellation Policy

Cancellation Policy  – Effective April 15, 2016

All patients are expected to keep their scheduled appointments.

If it is necessary that you cancel a scheduled appointment, we require that you notify our office no later than 3:00 p.m., the business day before the appointment. For example, if your appointment is on a Monday, you must call us by no later than 3:00 p.m., the previous Friday.

If you are not present at the scheduled time, without first notifying us as outlined above, we will add a $20 fee to your account. Three (3) such occurrences in a 12 month period will result in discharge from our practice.

It is truly our desire that our patients keep their scheduled appointments so that we can give them the medical care that they deserve. If this is just not possible, we are simply asking that you notify us so that someone else can take advantage of the office time.
